29 CFR 1926.403(b)(1): Examination. The employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees. On 4/24/2018, employees from TVH Exterior LLC Company were working on the side of a commercial building located at 1317 Scenic Rivers Blvd. in Salem, MO. Two employees were utilizing an electric drill to mix the base coat required for the side of the building. The power cord for the drill in use had been spliced and or repaired improperly in two areas. The employer did not ensure the electrical equipment was free from recognized hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On 4/24/2018, employees from TVH Exterior LLC Company were working on the side of a commercial building located at 1317 Scenic Rivers Blvd. in Salem, MO. Two employees were working on the side of a commercial building while working from a JLG Aerial Lift, Model # 400S. The employees were wearing personal fall arrest systems, but were not tied off to the basket.
