Instance 1: An employee was exposed to a fall hazard of 12ft while installing black paper from the 2nd tier fabricated frame scaffold. Instance 2: An employee was exposed to a fall hazard of 30ft while installing corner beads from the 4th tier fabricated frame scaffold. End frames were used as a means of access: On or about 4/25/2018, at 185 Pago Pago Drive West, in the city of Naples, employees were exposed to a fall hazard while using a 4-tier scaffold that lacked a ladder to access all tiers of the scaffold. Instance 1: An employee was exposed to a fall hazard of 12ft while climbing down the end frames from the 2nd tier of the fabricated frame scaffold. Instance 2: An employee was exposed to a fall hazard of 30ft while climbing down the end frames from the 2nd tier of the fabricated frame scaffold.
