29 CFR 1926.753(d)(2)(i): Materials being hoisted were not rigged to prevent unintentional displacement. a) Employees were exposed to being struck by a falling steel beam when hoisting a steel beam to the 6 story roof with the Cormach Crane (knuckleboom crane truck) and not securing the steel beam. The beam was placed on the forks of the boom truck without being secured to prevent it from sliding and falling off. Location: 4112 8th Ave, Brooklyn, N.Y; construction site. On or about 4-6-2018
