On April 18, 2018, the employer failed to report to OSHA a workplace related injury and in-patient hospitalization.  The employer is failing to meet OSHA requirements by reporting an in-patient hospitalization within 24 hours.  This most recent occurrence of this took place on April 25, 2018 at the facility located 4228 S. 72nd Street, Omaha, NE where on April 17, 2018 at approximately 11:50 am the employer had an employee injured while operating an electric pallet jack to the extent he was admitted and treated at the hospital.  The employer reported the incident on April 18, 2018 at approximately 6:04 pm.

This was most occurrence of this was evident during the inspection on April 25, 2018 of the facility located at 4228 S. 72nd Street, Omaha, NE, where the employer has employees operating three Crown electric pallet jacks as means of moving pallets of product throughout the facility without legible data plate as means of referencing the equipments lifting capabilities to prevent overloading the equipment.
