29 CFR 1926.502(d)(3): Dee-rings, used as part of a personel fall protection system, did not have a minimum tensil strength of 5000 pounds: a) On or about April 24, 2018 at 1246 West Saunders Road, Dothan: The employer failed to ensure an employee used the proper Dee ring on his harness to connect his snap hook in that the snap hook was attached to the hammer hold of the tool belt which exposed him to fall hazards of 12 feet 5 inches.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself: a) On or about April 24, 2018 at 1246 West Saunders Rd, Dothan: The employer did not ensure an employee using personal fall arrest were properly anchored in that his rope was wrapped around a support beam and the snap hook was connected back to the rope while installing bracing on the rafters which exposed them to falls of 12 feet 5 inches.
