29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:    a)  Warehouse    The employer did not conduct a workplace hazard assessment to determine if hazards were present that required the use of personal protective equipment (PPE).  On 04/03/18, an employee was splashed in the face and eyes when a mixing container tipped over.  The employee was not wearing a face shield.        Condition occurred on or about 04/03/2018.
