29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  The Landing Apartments, Northside of Building 12:  On or about April 26, 2018, and at times prior thereto, employees were exposed to fall hazards to the ground below, while installing rock from a scaffold which did not have base plates or mud sills below the support posts.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  The Landing Apartments, Northside of Building 12:  On or about April 26, 2018, and at times prior thereto, employees were exposed to fall hazards to the ground below, while climbing the scaffold end frames and cross braces to access the upper tiers of the scaffold to install rock on the exterior wall.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     (a) The Landing Apartments, Northside of Building 12:  On or about April 26, 2018, and at times prior thereto, employees were exposed to a fall hazard greater than 10 feet to the ground below while installing rock from the third tier of the scaffold that did not have guardrails installed.    (b) The Landing Apartments, Northside of Building 12:  On or about April 26, 2018, and at times prior thereto, employees were exposed to a fall hazard greater than 10 feet to the ground below while installing rock from the side bracket of the scaffold that did not have guardrails installed.

29 CFR 1926.454(a)(3): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the proper use of the scaffold, and the proper handling of materials on the scaffold, in order to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  The Landing Apartments, Northside of Building 12:  On or about April 26, 2018, employees were exposed to a fall hazard of greater than 10 feet to the ground level below while working from a scaffold without being trained to recognize the hazards.
