29 CFR 1910.1001(j)(5)(i): The building owner or employer identified previously installed asbestos containing materials (ACM) and/or presumed asbestos containing materials (PACM), and did not affix or post labels or signs so that employees would be notified of what material contains ACM and/or PACM: (a) Throughout Hangar 301 - On or about April 19, 2018, Civilian and Military workers were conducting their day to day job functions in their working and common areas that had asbestos containing pipe insulation and floor tile that contains <1% to 60% Chrysotile and 15% Amosite and the employer had not affixed or posted labels identifying the material as such. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1001(j)(3)(iii): Building and facility owner(s) did not inform employer(s) of employee(s); and employer(s) did not inform employee(s) who performed housekeeping activities in area(s) which contained ACM and/or PACM of the presence and location of ACM and/or PACM in such area(s) which might have been contacted during such activities: a) Throughout Hangar 301 - On or about April 19, 2018, DSC Contractor Custodian was responsible for housekeeping duties in a facility that has asbestos containing floor tile that contains 2% to 15% floor tile and the building and facility owner did not inform her of the presence, location, and quantity of ACM or PACM. b) Throughout Hangar 301 - On or about April 19, 2018, Tyonek Contractor Project Manager who occasionally mop the floor in a facility that has asbestos containing floor tile that contains 2% to 15% Chrysotile and the building and facility owner did not inform them of the presence, location, and quantity of ACM or PACM. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. 29 CFR 1910.1001(j)(7)(iv): The employer did not provide to all employees who perform housekeeping work in areas where ACM and/or PACM is present, an asbestos awareness training course covering the health effects of asbestos; and/or locations of ACM and/or recognition of ACM and PACM damage and deterioration and/or requirements in this standard related to housekeeping, and/or proper response to fiber release episodes: (a) Throughout Hangar 301 - Civilian and military workers were conducting their day to day job functions in their working and common areas that has asbestos containing pipe insulation and floor tile that contains <1% to 60% Chrysotile and 15% Amosite insulation and are required to clean up asbestos insulation that falls down onto the floor and the employer did not provide them with adequate asbestos awareness training. (b) Throughout Hangar 301 - On or about April 19, 2018, DSC Contractor Custodian was responsible for housekeeping duties in a facility that has asbestos containing floor tile that contains 2% to 15% floor tile and the building and facility owner did not provide them with adequate asbestos awareness training.
