(b) On or about April 26, 2018 and at times prior to that date, for the employees exposed to amputation hazards where the Accurshear, Hydraulic Metal Shear, model number 625012, serial number 5266 was operated in the machine shop at 406 Tower Street, Park River, North Dakota.
