29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about 4/25/2018, at 185 Pago Pago Drive West, in the city of Naples, employees were exposed to a fall hazard while using a 4-tier scaffold that lacked planking.   Instance 1: On or about 4/25/2018, at 181 W. Pago Pago Drive, in the city of Naples, an employee was exposed to a fall hazard while hoisting roofing material from the 4th tier of a fabricated frame scaffold.   Instance 2: On or about 4/25/2018, at 181 W. Pago Pago Drive, in the city of Naples, two employees were exposed to a fall hazard while using the 4th tier of the scaffold that lacked planking to access a ladder leading to the roof. End frames were used as a means of access:  On or about 4/25/2018, at 181 W.
