Instance 1: On or about 4/25/2018, at 181 W. Pago Pago Drive, in the city of Naples, an employee was exposed to a fall hazard while hoisting roofing material from the 4th tier of a fabricated frame scaffold. Instance 2: On or about 4/25/2018, at 181 W. Pago Pago Drive, in the city of Naples, two employees were exposed to a fall hazard while using the 4th tier of the scaffold that lacked planking to access a ladder leading to the roof.
