29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) 1215 Warrington Ave, Norfolk, VA 23507, U.S.S. Mahan: On or about June 14, 2018, and times prior thereto, employees were exposed to noise levels in excess of the allowable 8-hour time weighted average (TWA) of 85 dBA. This violation was observed on or about June 14, 2018, where noise sampling performed up to 467 minutes indicated that an employee was exposed up to an approximately 70.8 percent noise dose which is equivalent to 87.7 dBA, exposing the employee to potential hearing loss. Zero exposure was assumed for the 13 minutes not sampled. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.41(a)(2): Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries. The establishment had 20 or more employees but fewer than 250 employees at some time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, but did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form: a) Continental Tide Defense Systems, Inc., 1457 Miller Store Rd, Ste 104, Virginia Beach, VA 23455: On or about April 27, 2018, and times prior thereto, the employer failed during calendar year 2017 to electronically submit information from the OSHA Form 300A or equivalent by December 15, 2017. The establishment employed approximately 26 employees and was classified in 336611 NAICS during calendar year 2016. No abatement certification or documentation required.
