29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:   a) Molding Department - On April 26, 2018, the employer did not ensure that the energy control procedures were specific to the equipment such as the Hella Westgate machine #107 and clearly outlined the following: a specific statement of the intended use of the procedure (servicing, maintenance, and/or un-jamming), procedural steps for shutting down, isolating and securing the machine to control hazardous energy (electrical, and pneumatic), procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices, tagout devices and other energy control measures.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Molding Department - On or about April 17, 2018, an employee was exposed to machine hazards associated with moving parts while unjamming a metal strip on the Hella Westgate Machine #107, Die #1.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employee from machine servicing hazards.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Molding Department - On or about April 17, 2018, the employer did not ensure that employee(s) performing servicing and maintenance activities such as unjamming a metal strip on the Hella Westgate Machine #107, Die #1, were trained as an authorized employee.  The employee performed this servicing and maintenance task without the knowledge and skills needed for the safe application, usage, and removal of the energy controls.
