29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems.  A) Bock & Bros Roofing & Snow Removal LLC on or about April 26, 2018 at the address of 8709 Big Cypress, Sylvania, OH: The employer did not ensure employees were protected from falls while installing new roofing materials (asphalt shingles) to a residential type structure.  The employees were exposed to a potential 12 foot fall to the level below.                    Bock & Bros Roofing & Snow Removal LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1282471, citation number 1, item number 2a, and was affirmed as a final order on February 2, 2018, with respect to a workplace located at 1802 Sylvania Avenue, Toledo, Ohio.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder.  A) Bock & Bros Roofing & Snow Removal LLC on or about April 26, 2018 at the address of 8709 Big Cypress, Sylvania, OH: The employer did not ensure the side rails of the 24 feet aluminum extension ladder, extended at least three feet above the landing surface. One employee was exposed to falls of up to 12 feet while using the ladder to access the east side of the roof.   B) Bock & Bros Roofing & Snow Removal LLC on or about April 26, 2018 at the address of 8709 Big Cypress, Sylvania, OH: The employer did not ensure the side rails of the 24 feet aluminum extension ladder, extended at least three feet above the landing surface. One employee was exposed to falls of up to 12 feet while using the ladder to access the west side of the roof.  Bock & Bros Roofing & Snow Removal LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1282471, citation number 1, item number 2b, and was affirmed as a final order on February 2, 2018, with respect to a workplace located at 1802 Sylvania Avenue, Toledo, Ohio.
