29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: On or about April 20, 2018, employees were exposed to hazardous electrical and pneumatic energy sources while performing service and maintenance activities on the Hytrol Conveyor system and the energy control procedures did not specify where to apply lockout devices or provide instructions to verify that the energy was controlled and the machine was rendered safe.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: On or about April 20, 2018 employees required to perform service an maintenance activities on conveyor systems and other equipment were exposed to entanglement hazards and the employer did not conduct training that ensured employees understood how to safely de-energize and isolate hazardous energy sources on equipment they were required to repair and maintain.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1) through (6): On or about April 20, 2018 employees were exposed to machine hazards associated with moving parts and in-running nip points during servicing and maintenance, and trouble-shooting activities on the Hytrol Conveyor in the warehouse area. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
