29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about 4/27/2018, on the roof of the building at 5740 Steeple Chase Blvd, Cumming, GA: Employer failed to ensure the personal fall arrest systems provided to the employees observed working on and near the edge of the roof with unprotected sides and edges at height up to 24 feet were inspected prior to use. The personal fall arrest systems inspected was noted with defects such as but not limited to damage lanyards, manufacture identification tags torn off, and black tape used on the equipment.
