Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. a) LPG container storage: On April 26, 2018, and at times prior, the employer exposed employees to burn hazards while removing and replacing LPG containers on forklifts, in that proper skin protection was not provided.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) Production Area, Bagging Station Number 2: On or about April 26, 2018, and at times prior, the employer exposed employees to struck-by and crushing hazards, in that the movable arm for the bagging station was not properly guarded while operating the bail bagger.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) Warehouse: On or about April 26, 2018, and at times prior, the employer exposed employees to struck-by and crushing hazards when operating Forklift (Hyster Model S55FTS Serial Number H187V02969N), in that the forklift had an attachment (Loron Clamp Model No. 112914 Serial No. 201509016) without a label identifying the new lifting capacity.
