On or about April 23, 2018, during the installation of roofing material on a residential single family home located at 207 Massachusetts Ave SE, Massillon, Ohio 44646, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of a fall height of approximately 9.4 feet above the ground.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about April 23, 2018, during the installation of roofing material at a job site located at 207 Massachusetts Ave SE, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 9.4 feet by utilizing a fall protection system. Cochran Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 950442, citation number 01, item number 01 and was affirmed as a final order on 3/8/2017, with respect to a workplace located at 4401 Cliffview Dr., Independence, Ohio 44131.
