29 CFR 1910.106(e)(6)(ii): Class I flammable liquid(s) were dispensed into containers without electrically interconnecting the nozzle and the container, vehicles, or equipment:         On or about April 26, 2018, and at times prior thereto, in cabinet shop lacquer thinner having flashpoint less than zero degrees F was dispensed into a smaller container without being effectively  bonded to electrically interconnect the containers to prevent static discharge during dispensing operation exposing employees to a fire hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:         On or about April 26, 2018, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances:       1) Paint area in Building 6, a washing chain-link conveyor was being used without guards provided for in-running nip points.       2) At the belt sander in maintenance shop, a belt sander was being used without guards provided to protect employees from contacting the rotating parts while in operation.     Employees were exposed to  amputation and caught in hazards.

Employees were exposed to flying fragments in the event of an exploding wheel.
