29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Jobsite, building 3030:  On April 27, 2018 and at times prior thereto, two employees installed drywall materials from an aerial lift.  Neither of the employees was wearing a personal fall restraint system.  This exposed them to a potential fall of 15 to 25 feet to the ground below.       Note) As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    Note) Certification of corrective action is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Jobsite, building 3030: On April 27, 2018 and at times prior thereto an employee performed sheathing activities on a 6 in 12 pitched balcony roof.  The personal fall protection harness the employee was wearing was not attached to a lanyard or anchor point.  This exposed the employee to a 30 foot fall to the ground below.      b) Jobsite, building 3030: On April 27, 2018 and at times prior thereto the guardrail systems on the second floor stairway landings were not installed properly.  The guardrail did not extend to the corner where the stairways meet the landings.   This created a tripping hazard at the corner of the stairways and landings.      c) Jobsite, building 3022: On April 27, 2018 and at times prior thereto a temporary guardrail system was not installed on an open sided floor located on the second floor.  An employee used this area for access to the second floor.  This exposed him to a 10 foot fall the concrete below.       d) Jobsite, building 3022: On May 9, 2018 and at times prior thereto an employee performed framing activities from the external top plate.  The personal fall protection harness the employee was wearing was not attached to a lanyard or anchor point.  This exposed him to a 30 foot fall to the ground below.     e) Jobsite, building 3030: On May 9, 2018 an employee walked on a 6 in 12 pitched roof.  The employee was not performing any specific construction activities at the time.  The employee was not wearing a personal fall protection system.  This exposed him to a 30 foot fall to the ground below.    Note) Certification and documentation of corrective action are required for this item.
