29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut.        In the Wall Build Department, the employer failed to ensure the guard on the 10" Makita saw was in good condition and was operable, exposing employees to amputation hazards.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.    The employer failed to report to OSHA a work-related hospitalization, a hospitalization occurred on April 6, 2018 and was not reported to OSHA until April 13, 2018.
