29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 30, 2018, employees walking/ working from the roof of a commercial building were not protected from a fall, exposing employees to a fall of 10.9 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 30, 2018, employees used an extension ladder that did not extend three feet past the roof they accessed, exposing the employees to a fall.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
