(a) On or about April 20, 2018 at the 113 N. Western Ave., Waupaca jobsite, a Vulcan Ladder System used to access a commercial roof which did not extend at least three feet past the landing. (b) On or about April 30, 2018 at the 113 N. Western Ave., Waupaca jobsite, a Cosco Multi-Use Ladder System used to access a commercial roof which did not extend at least three feet past the landing.
