29 CFR 1910.22(d)(2): Hazardous conditions on walkingworking surfaces were not corrected or repaired before an employee uses the walking-working surface again. If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired: a) Rear of the shop: On or about April 26, 2018 and at times prior employees were exposed to trip hazards in that the cover plate on a hole in the asphalt was moved exposing part of the hole itself.

29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: a) Shop - On or about April 26, 2018 and at times prior employees were exposed to struck by hazards in that an employee was involved in an accident resulting in property damage from a forklift and the employer did not pull the operator's license and retrain the operator.
