29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a.) Blast Area:  An employee sand blasting metal material was exposed to 215.8% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 95.5 dBA during the 450 minute sampling period on May 1, 2018.The exposure calculation included a zero increment for the thirty minutes not sampled. The employer has not established a written program and the employee had not received an annual audiogram.  b.) Plasma Area: An employee that is plasma coating metal material was exposed to 272.5% of the permissible daily exposure of 85 dBA (8-hour time weighted average) or an equivalent sound level of 97.2 dBA during the 450 minute sampling period on May 1, 2018.The exposure calculation included a zero increment for the thirty minutes not sampled. The employer has not established a written program and the employee had not received an annual audiogram.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a.) Dip Spin Area: On or about 4/12/18, and at times prior, the employer exposed employees to chemical hazards, in that plumbed eyewash stations for quick drenching of the eyes were not provided to employees while working with corrosive hazardous material such as Molykoter (oxylic acid).
