Employees are required to walk in close proximity to the unguarded section of the drainage channel system located on the floor of the shop area where the recycle water drains into. The grates which covers the drainage was missing, exposing an open section area approximately 20ft. long x 2ft. deep x 8ft. wide, where an employee can accidentally trip and fall when walking in close proximity to the area, on or about  May 01, 2018.    b.  180 Industrial Park Circle, Lawrenceville, GA (Shop Area) - Employees were exposed to fall hazard.  Employees are required to walk and work in close proximity to the unguarded collected recycle water pit approximately 2'8" deep x 3ft wide, exposing employees to trip and/or fall when walking and/or working in close proximity to the unguarded pit, on or about May 01, 2018.

The grinder guard was removed and/or missing, exposing employees to flying particles and struck by hazard by the rotating blade, on or about May 01, 2018.
