29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about April 24, 2018, at the worksite located at 2605 NW 75th Avenue, Miami, Florida 33122, the employer added a mancage basket and pallet to a Mitsubishi forklift in order for an employee to perform work on an overhead garage door. The added device effects the safe operation of the forklift in that the added equipment changes the center-of-gravity of the forklift.
