29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    a) Warehouse at 7251 Klier Drive in Fairview, PA, Aisle 25-26.  On or about April 28, 2018, an employee fell 30 feet to the concrete floor while handling a queen size mattress.      b) Warehouse at 7251 Klier Drive in Fairview, PA.  On or about May 1, 2018, and times prior thereto, employees were not protected from falling 30 feet to the lower level.  When handling furniture items that were large and/or bulky, employees disconnected lanyards from anchorage points to load and unload the items from the storage racks, up to 30 feet above the floor.
