29 CFR 1926.503(b)(1): The employer did not verify fall protection training by preparing a written certification record. (a) Job site: 477 Frye Farm Road, Latrobe, PA 15650. On or about April 27th, 2018 the employer did not maintain fall protection certification records for the employees on site that utilized fall protection while working to replace a residential roof.

29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable. a) Job site: 477 Frye Farm Road, Latrobe, PA 15650. On or about April, 27th 2018 employees were working to replace shingles on a residential roof. The employees were using nail guns to install the shingles on the roof. Some of the employees on the roof were not wearing eye protection while operating the nail guns.
