29 CFR 1926.501(b)(15):     "Walking/working surfaces not otherwise addressed." Except as provided in 1926.500(a)(2) or in 1926.501 (b)(1) through (b)(14), each employee on a walking/working surface 6 feet (1.8 m) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system.    On or about 4/26/2018, employees from Smith and Collier Construction, LLC were installing trusses on an addition of a commercial building located at 717 North Meramec Street in St. James, MO.  They were not utilizing a guardrail system, a safety net system, or personal fall arrest systems.  The employer and two employees were attaching bracing to trusses approximately fourteen feet above the ground.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.    On or about 4/26/2018, employees from Smith and Collier Construction, LLC were installing trusses on an addition of a commercial building located at 717 North Meramec Street in St. James, MO.  The employer and two employees were attaching bracing to trusses approximately fourteen feet above the ground without utilizing a guardrail system, a safety net system, or personal fall arrest systems.  The employer had not trained the employees so they were able to recognize fall hazards and the procedures necessary to reduce or eliminate hazards.

29 CFR 1926.602(d):     The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):    Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178 and as referenced by the note contained in 1926.602(d).    On or about 4/26/2018, employees from Smith and Collier Construction, LLC were installing trusses on an addition of a commercial building located at 717 North Meramec Street in St. James, MO.  The employer had not ensured their employees were competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the required training.  The employees, who had not completed the required training, had operated a Sky Trak Powered Industrial Truck, Model 6036, Serial # 0160019953, while working on a building located at 717 N. Meramec Street in St. James, MO.
