29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Press Dept. : 1) A feeder shoe cam pinchpoint of Stokes Press 1T4 was not adequately guarded. 2) A pinchpoint created by the eccentric and the body of the Stokes Press 1T5 was not guarded. 3) The left sides of the die areas of Stokes Presses 1T4 and 1T5 were were not guarded. 4) The guard at the part discharge chute section of Stokes Press 1T4 was not equipped with an interlock device to prevent the machine from starting with the guard open and to cause the machine to stop when the guard is opened. A worker was injured while cleaning the die area while the press was running.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Worksite: The employer did not report a March 26, 2018 injury to OSHA where an employee had a finger amputation injury at the Stokes 1T4 Press while cleaning the machine while the press was running.
