29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): On or about April 24, 2018, one employee walked behind the conveyor belt in the warehouse while lock-out had been removed for testing the machine, exposing employee(s) to the hazards of being caught in a rotating projecting shaft.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends did not present a smooth edge and end and projected more than one-half the diameter of the shaft without being guarded by nonrotating caps or safety sleeves.
