29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A-D): a) On or about April 24, 2018, the lockout procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and control hazardous energies on the 16 Foot Straw Blanket Counter Presser #36. Employees were exposed to crushing hazards due to the pneumatic and gravitational energy sources not being isolated while performing a maintenance and servicing task. American Excelsior Company was previously cited for a violation of this Occupational Safety and Health standard 1910.147(c)(4)(ii) which was contained in OSHA inspection number 1244998, citation number 1, item number 1a and was affirmed as a final order on November 15, 2017, with respect to a workplace located at 831 Pioneer Avenue in Rice Lake, Wisconsin.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) On or about April 24, 2018, the employer did not assure that operators of the 16 Foot Straw Blanket Counter Presser #36 were adequately trained in the skills required for the safe application, usage, and removal of the energy controls. Employees were not trained or instructed to isolate the pneumatic and gravitational energy sources while performing maintenance and servicing tasks. American Excelsior Company was previously cited for a violation of this Occupational Safety and Health standard 1910.147(c)(7)(i), which was contained in OSHA inspection number 903792, citation number 1, item number 5d and was affirmed as a final order on September 12, 2013 with respect to a workplace located at 180 Cleveland Road in Norwalk, Ohio.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about April 24, 2018, the employer did not ensure that energy control application steps to control the electrical, pneumatic and gravitational energy sources were implemented prior to maintenance tasks were performed on the 16 Foot Straw Blanket Counter Presser #36. An employee suffered a crushing injury when a second employee lowered the stitcher arm while a maintenance and servicing activity was being performed. American Excelsior Company was previously cited for a violation of an equivalent Occupational Safety and Health standard 1910.147(d)(3) and (d)(4)(i),which were contained in OSHA inspection number 1244998, citation number 1, item number 1b and 1c and were affirmed as a final order on November 15, 2017, with respect to a workplace located at 831 Pioneer Avenue in Rice Lake, Wisconsin.
