29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A-D):      a)  On or about April 24, 2018, the lockout procedures failed to clearly identify all of the specific steps to be followed by employees to isolate and control hazardous energies on the 16 Foot Straw Blanket Counter Presser #36.  Employees were exposed to crushing hazards due to the pneumatic and gravitational energy sources not being isolated while performing a maintenance and servicing task.       American Excelsior Company was previously cited for a violation of this Occupational Safety and Health standard 1910.147(c)(4)(ii) which was contained in OSHA inspection number 1244998, citation number 1, item number 1a and was affirmed as a final order on November 15, 2017, with respect to a workplace located at 831 Pioneer Avenue in Rice Lake, Wisconsin.
