On or about May 1, 2018, employees were exposed to eye hazards while installing roofing materials on a residential home located at 8455 Cherry Hill, Broadview Heights, Ohio 44147. The employer did not ensure employees using air powered nail guns wore protective safety glasses.

The employer did not ensure the ladder extended three feet above the landing area.
