29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph: On or about 5/1/2018, at 18831 Biscayne Blvd, in the city of Aventura, an employee positioned a 12ft fiberglass step ladder on top of a 5ft rolling scaffold to access a commercial roof exposing himself to a fall hazard of more than 10ft. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
