29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, shall: Be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level: a) On or about May 02, 2018, at the above referenced worksite, the employer failed to ensure employees properly wore Personal Fall Arrest Systems (PFAS). Employees at the worksites wore PFAS harnesses losely and in some instances without the leg straps secured, thereby affecting the fall arresting capabilities of the harnesses. Employees were thereby exposed to injury from falls in excess of 6 feet. There is no abatement certification of documentation required for this item.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) On or about May 02, 2018, at the above referenced worksite, employees were not properly trained in use of Personal Fall Arrest Systems and wore their fall protection harnesses in a loose fashion, thereby exposing employees to injury from arrested fall in excess of 6 feet. b) On or about May 02, 2018, at the above referenced worksite, employees were not adequately trained in fall protection use while while transitioning between lifelines, thereby exposing employees to falls in excess of 6 feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) On or about May 02, 2018, at the above referenced worksite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing employees to the hazards associated with eye injury. Theisen Roofing & Siding Co., Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1110736, citation number 1, item number 1, and was affirmed as a final order on February 08, 2016 with respect to a workplace located at 1833 - 1839 Kerrybrook Ct, Sycamore, IL 60178. There is no abatement certification or documentation required for this item.
