29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s); a) Jobsite: On May 2, 2018, and a times prior thereto the controlling employer Wright Brothers, The Building Company, did not ensure through effective frequent and regular inspections that their employees and sub-contractors were using fall protection systems when needed. Wright Brothers employees and a sub-contractor worked at a wall opening on the west end of the second floor, unloading construction materials from a rough terrain forklift into the building. No fall protection system was being used. This exposed them to a 12 foot fall hazard. b) Jobsite: On May 2, 2018 and a times prior thereto the controlling employer Wright Brothers, The Building Company did not identify hazards through an effective frequent and regular inspection program. Hazards included, but may not be limited to, sub-contractors not wearing head protection when needed. c) Jobsite: On May 2, 2018 and a times prior thereto the controlling employer Wright Brothers, The Building Company did not identify hazards through an effective frequent and regular inspection program. Hazards included, but may not be limited to, handrails were not in place on active stairways, and portable fire extinguishers were not conspicuously located through-out the project. d) Jobsite: On May 10, 2018 and a times prior thereto the controlling employer Wright Brothers, The Building Company did not identify hazards through an effective frequent and regular inspection program. Hazards included, but may not be limited to sub-contractor employees not using approved methods for cutting cultured stone which contained silica. Note) Certification of corrective action is required for this item.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) Second floor: On May 2, 2018 and at times prior thereto employees who worked at a wall opening on the west end of the second floor were exposed to a 12 foot fall hazard. The employees were unloading construction materials from a rough terrain forklift into the building. No fall protection system was being used. Note) Corrected during the inspection.

Employees of the controlling employer, Wright Brothers, The Building Company, and employees of various sub-contractors used the stairways for access to the upper level floors, along with moving construction tools and materials between floors.
