29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section:    The employer is failing to protect employees from caught-in and amputation hazards while servicing and performing maintenance on equipment including by not limited to the Amada turret punch press, Trumpf laser, and Trumpf press brake.  This was most recently documented to occur on or about May 10, 2018 and dates prior in the workplace located at 14242 C Circle, Suite B, Omaha, NE 68114.  Machine specific lockout/tagout procedures have not developed for the these and other types of equipment that explain hazardous energy sources, the proper sequence for de-energization, and specific energy isolation points for multiple energy sources such as the compressed air lines, and electrical feeds.

29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area:    The employer is failing to train employees on the physical and health hazards of hexavalent chromium in work areas where the contaminant could be present while employees weld on stainless steel.  This was most recently documented to occur in the workplace located at 14242 C Circle, Suite B, Omaha, NE 68114 on or about the time frame of three weeks prior to April 26, 2018 when employees performed welding of stainless steel metals.

The employer is failing to evaluate employees' exposure to hexavalent chromium when they perform welding operations on stainless steel.  This was most recently documented to occur in the workplace located at 14242 C Circle, Suite B, Omaha, NE 68114 on or about the time frame of three weeks prior to April 26, 2018 when employees performed welding of stainless steel metals.
