29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:      Instances including, but not limited to, the following:    a) Job site, 2001 Park Manor Boulevard, Robinson Township, PA: On or about May 2, 2018, during steel erection activities, the employer did not ensure that fall protection systems/devices were in use while on a canopy, exposing employees to fall hazards of up to approximately 17 feet.     b) Job site, 2001 Park Manor Boulevard, Robinson Township, PA: On or about May 2, 2018, during steel erection activities, the employer did not ensure that fall protection systems/devices were in use while on the main (top) roof of the facility, exposing employees to fall hazards of up to approximately 17 feet.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:      a) Job site, 2001 Park Manor Boulevard, Robinson Township, PA: On or about May 2, 2018, during steel erection activities, the employer did not ensure that proper ladder safety practices were followed where an extension ladder was used to get up/down between roof levels in that the ladder was not extended at least 3 feet above the landing and it was not secured and a grab bar was not provided, exposing employees to fall hazards of up to approximately 17 feet.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:      a) Job site, 2001 Park Manor Boulevard, Robinson Township, PA: On or about May 2, 2018, during steel erection activities, the employer did not ensure that a ladder or stairway was provided between work levels that were determined to be over 3 feet in distance from one another, exposing employees to injuries such as lacerations and strain/sprains.
