On 5/02/2018, Adelso Somazo DBA Adelso Somazo was installing shingles on a roof of a residential building located at 217 Old Ballwin Road in Ballwin, MO. The owner and three employees were observed not utilizing guardrail systems, a safety net system, or personal fall arrest systems. They were working on a 5/12 sloped roof approximately nine feet above the ground without fall protection, exposing them to a fall to the ground below.
