29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.  a. On or about May 2, 2018, at the above addressed jobsite, employees accessing a scaffold and ladders were exposed to trips and falls at the ground level because siding debris was not kept clear from the area.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a. On or about May 2, 2018, at the above addressed jobsite, an employee siding a building from a pump jack scaffold, sixteen (16) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a. On or about May 2, 2018, at the above addressed jobsite, employees re-siding a house using a pump jack scaffold were not provided training on how to minimize fall hazards when working from a pump jack scaffold, thereby exposing the employees to the hazards associated with falls.
