On or about 5/4/18 in the following locations: a) Building 3, Guillotine Department, numbers A-D the employer allowed employees to operate the guillotine machines without using two-hand tripping mechanisms. b) Building 3, Line 6 Conveyor belt, the employer allowed employees to operate and feed plastic bundles onto the conveyor belt that carries the bundles 35' and empties them into the shredder where the operators/feeders were not provided with point of operation guarding to prevent them from being carried into the shredder.
