29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. Site: 77 Clackson Avenue Brooklyn, NY On or about 5/2/18 a) Employees used a job made ladder to access foundation of building. The ladder did not extend 3 feet past the intended landing. The employees climbed down from the ground level to the top of the foundation wall then used ladder to reach their work area.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Site: 77 Clarkson Avenue Brooklyn, NY On or about 5/2/18 a) Employees standing on wall and on a steel beam were not utilizing any fall protection. They were exposed to a fall hazard from approximately 10 feet. Second Repeat Magellan Concrete Structures was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1) which was contained in OSHA inspection number 1187756, citation number 2, item number 1 and was affirmed as a final order on 8/21/2017 with respect to a workplace located at 321 Wythe Avenue Brooklyn, NY 11249
