29 CFR 1910.27(b)(2)(iv): The employer did not ensure the rope descent system was inspected at the start of each workshift that it was to be used. The employer did not ensure damaged or defective equipment was removed from service immediately and replaced: On or about May 3, 2018, and at times prior thereto, employees were exposed to fall hazards, while using an unfit SkyGenie and job-made rope descent chairs that were found to be unfit and were not immediately removed and replaced such as those created by wear or deterioration: a) South side, descent area: On or about May 3, 2018, and at times prior thereto, employees were exposed to a fall hazards from Sky Genie which descent chair showed evidence of chipping/broken and excessive wear. b) South side, descent area: On or about May 3, 2018, and at times prior thereto, employees were exposed to a fall hazards from descent chair which had illegible markings, missing labels and excessive soiling such as mold and screws embedded into the descent chair to hold carpet on seating area in place. c) South side, descent area: On or about May 3, 2018 and at times prior thereto, employees were exposed to fall hazards when shock absorbed lanyard utilized was wrapped around with black electrical tape which may keep it from deploying. d) South side, descent area: On or about May 3, 2018 and at times prior thereto, employees were exposed to fall hazards when utilizing Singing rock waist belt being utilized showed evidence of rust at the buckles, D ring and hook.
