The employer is failing to correct the deficiency of improper identification of ammonia process components in the following instances:  A) Multiple valves on associated transfer piping for the Warrender pump for the Bulk Ammonia Receiver Vessel were not labeled, including valves numbers 010198, 010199, 010200, 010201, 010202, 010203, 010205, 010206, 010207, 010208, 010209, 010210, and 010211.  B) Valve#003195 on associated piping for the LTA-1 Accumulator was not labeled.  These hazards were last observed on August 23, 2018.

29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience.  The employer is failing to ensure that inspections of all valves and process equipment is of a frequency to ensure functionality. Valve(s), ground level and in the pit for process piping/equipment associated with Accumulator - 4973 in the South Engine Room, were completely or partially covered in ice. Valve identification tags were also encased in ice. Inspection and de-icing schedule indicated accumulator only monthly checks/de-icing. This hazard was last observed on August 23, 2018.
