For Insulated lines - minimum distance of 3 feet when line is less than 300 V  a) Exterior of Building - Scaffold Driveway - Vandelinda Ave.: Employee engaged in siding activities from a pump jack scaffold platform that was less than one foot from a live electric residential service, was exposed to electric hazards, on or about 5/2/18.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a) Exterior of Building - Scaffold Driveway - Vandelinda Ave.: Employee without proper means of fall protection, engaged in siding activities from a pump jack scaffold platform, was exposed to fall hazards of approximately 11 feet, on or about 5/2/18.
