29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. a. Perkins Machine Company power press model 75-S, serial #40674; On or about April 27, 2018, an employee was exposed to crush-by and amputation hazards resulting in the partial amputation of three fingers on the employees left hand. The employer did not ensure that point of operation devices were worn and/or properly adjusted.
