Perkins Machine Company power press model 75-S, serial #40674; On or about April 27, 2018, an employee was exposed to crush-by and amputation hazards resulting in the partial amputation of three fingers on the employees left hand. The employer did not ensure that point of operation devices were worn and/or properly adjusted.
