29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). On or about May 2, 2018, employees utilizing a JLG Skytrak while performing window installation operations from a "field modified" job box were exposed to a fall hazard when the employer failed to ensure the employees were adequately trained. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about May 2, 2018, employees performing window installation operations were exposed to a fall hazard when the employer failed to ensure a "field modified" job box had been certified in writing by the manufacturer or by any other equivalent entity. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
