29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) would be met:  a)  Facility: The employer did not develop and implement a written hazard communication program for the chemicals used by employees, such as, but not limited to Zerex Original Antifreeze coolant, Wearever Brake Fluid,and Rustek Lubricant, on or about 5/4/18.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a)   Facility: The employer did not provide training as part of the hazard communication program for the chemicals used by employees, such as, but not limited to, Zerex Original Antifreeze coolant, Wearever Brake Fluid,and Rustek Lubricant on or about 5/4/18.

29 CFR 1910.134(c)(2)(i): Where the employer provided respirators for employees to use voluntarily, the employer  did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"):    a) Facility: Employee(s) were not provided with Appendix D when 3M 8511N95 filtering face piece respirators were provided for voluntary use, as observed on 5/4/2018.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
