Site: 110 Greenpoint Avenue, Brooklyn, NY; Apt #2R    On or about 5/4/2018;    a) A Ryobi table saw used by employees on site did not have a guard protecting the saw blade.  Employees used this table saw to rip the wood pieces when installing wooden flooring.
