Alex Properties, Inc. dba Alex Restoration and Masonry Repairs, 121 West 10th Street, Erie, PA 16501, on or about May 4, 2018 and times prior thereto: Employees were operating equipment including, but not limited to, gasoline powdered concrete saws for masonry cutting operations. The Kidde multi-purpose dry chemical fire extinguisher (M/N: FA 5-1, S/N: BJ-915016) located in the pick-up truck near the work operations was not charged and ready for use.
